RESOLUTION APPROVING THE DONATION AMOUNTING TO

P___________PER PARENT AS PTA HOMEROOM PROJECT OF GRADE 7
_________, S.Y. 2018- 2019.

Whereas, the School Year 2018-2019 had started on June 4, 2018;

Whereas, we had a Homeroom PTA meeting on June ______ and we have
seen some problems/ needs of our new classroom;
Whereas, in order to address the problems and needs of the classroom,
MR./MRS _______________ had suggested to collect from each
parent an amount of ____________________ as Homeroom PTA Project
of Grade 7_______, S.Y. 2018-2019;
Whereas, it was duly seconded by all the members present and have agreed
to collect the said amount as soon as possible;
Whereas, the said amount to be collected will be used to purchase
_____________________ as Homeroom PTA Project this school year;
Whereas, the said amount to be collected is only a donation and on voluntary
basis;
Resolved further that copies of this resolution be forwarded to the office of the
Principal and PTA Homeroom President for the information and guidance.
