I am writing this letter to request the issuance of the Deed of sale of the Leased
Company Car by Judy Araya Martinez under Charmaine Jane Araya Martinez.
The Ford Ecosport with plate number IK5424 which is previously being leased by
Judy A. Martinez has been sold to her ownership upon her retirement from the
company; UCPB SAVINGS BANK. However, she may not be able to take ownership
of the said vehicle due to her passing aways last April 8,2019. In effect, I Charmaine
Jane A. Martinez, her daughter will be taking the car ownership in lieu of her. I am
writing this letter as a formal request to issue the Deed Of Sales for the Vehicle under
my name and ownership.
