1. A certified true copy of the BIR Ruling No. ______ dated __________
that is the subject of the request for review by the adversely affected party,
________; and,

2. A certified and authenticated duplicate copy of the complete docket

and records on file with the Bureau of Internal Revenue.

We further state that the copy of the taxpayer's request for review of the
above-stated Ruling has been endorsed by the Office of the Commissioner to, and
is on file with this Office.
