The student and Training Place Host will come to an agreement on how the student will be paid for duties performed on site. The student will require the Training Place Host to sign off on their accumulated hours on a monthly basis as evidence for hours worked. As the Training Place Host Tunderstand that I must provide a safe working environment. As the Training Place Host, I understand the student is being hired as an employee and that the student should be licensed and insured prior to starting the practicum hours. Therefore, I hereby waive all action, claim or demand that I may hereafter have against Makami College Ine. by reason of damages to equipment or property as a result of, or in any way rising from the agreement.
