North Carolina Department of Labor Inspection Number: 318163409 Occupational Safety and Health Division Inspeetion Date(s): 4/10/2019 - Issuance Date: 10/8/2019 Company Name: PSNC Energy DBA Dominion Energy North Carolina Inspection Site: 117N Duke Street, Durham, NC 27701 Citation 01 Item 001 ‘Type of Violation: Serious NCGS 95-129(1): The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or were likely to cause death or serious injury or serious physical harm to his employees in that employees were exposed to: fire/explosion hazards 4) Job site: 115 N Duke Street, near front of coffee shop - on 4/10/19, a locator/first responder was exposed to fire/explosion hazards due to ineffective response procedures which allowed the employee to respond to a gas leak assignment without wearing Personal Protection Equipment (PPE) before the assessment of the gas leak arca, b) Job site: 115 N Duke Street, near front of coffe shop - on 4/10/19, a locator/first responder was exposed to fire/explosion hazards due to ineffective response procedures which allowed the employee to park his company vehicle near the gas leak. Abatement: Instance (a)- The employer should develop effective procedures for the locator/first responder following the guidelines from the “Emergency Response Handbook” by USDOT (2012), for compressed natural gas, and PSNC/Dominion’s Safety Data Sheet (SDS) for natural gas which have guidelines for exposure control and personal protection. On both resources, employees exposed to a compressed natural gas spill, are required to wear a positive-pressure self-contained breathing apparatus (SCBA) and protective clothing (such as PBO/Kevlar coverall flash suit) before entering the hazardous area, Citation and Notification of Penalty Page 10 0f 12 (OSHA-2 (Rev 1/14) Instance (b) - On the “Emergency Response Handbook” by USDOT (2012), indicates that the ‘evacuation area for first responders responding to a compressed natural gas spill is located at approximately of 800 meters (1/2 mile) away from the gas leak; the employee’s vehicle needs to be parked somewhere near this distance.
