Good day! We are conducting our research about the insights of parents with children diagnosed with
autism on the perception of Fidelis Senior High Grade 11 Science, Technology, Engineering and
Mathematics (STEM) on people with Autism Spectrum Disorder. In regards to this, may we ask the
permission to conduct our interview on Fidelis Senior High Grade 11 STEM students. The participants of
our study were determined using purposive and quota sampling. We are aiming to start our interview on
March 11 – 15, 2019. We will execute it during on our Research 1 period, vacant time and after class.
Rest assured that all data collected will be confidential and would not be included in our research without
the permission of our respondents.

Dear Parents,

Good Day! We are Grade 11 STEM students from Fidelis Senior High, conducting a research
about the insights of parents with children diagnosed with autism on the perception of Fidelis
Senior High Grade 11 Science, Technology, Engineering and Mathematics (STEM) on people
with Autism Spectrum Disorder. This is a questionnaire regarding your insights on the
perceptions of Fidelis Senior High STEM students regarding people with ASD. Rest assured that
all data collected will be confidential and would not be included in our research without the
