Relative to the Memorandum from the PLEB ~ Program Management Office (PLEB-PMO)
dated October 3, 2019 re Submission of Consolidated Data and Progress Report on PLEB Monitoring
and Evaluation for 2019, kindly attach scanned copies of Executive Orders, Resolutions and
Ordinances pertaining to the creation and composition of the PLEBs along with photo
documentations (scanned copies) upon your submission. For the submission of the said Monitoring
Tool, kindly use this google drive link http://bit.ly/R7PLEB-MET and please disregard the link
provided by the PLEB-PMO on the attached memorandum as it is intended for the regional
submission.
Deadline of the submission of the Monitoring and Evaluation Tool and its necessary
attachments will be on October 16, 2019 as agreed upon during the conduct of the Final Briefing on
the PODS last October 2, 2019 at Tagbilaran City, Bohol.
For LGUs that are still in the process of reorganization, incumbent PLEB Members are advised
to participate in the accomplishment of the Monitoring Tool.

SUBJECT: SUBMISSION OF CONSOLIDATED DATA AND PROGRESS REPORT
ON THE PEOPLE'S LAW ENFORCEMENT BOARD MONITORING
AND EVALUATION FOR 2019

This refers to the Memorandum of Secretary Eduardo M. Afio, dated September 09,
2019 regarding the conduct of PLEB Monitoring and Evaluation for 2019. The deadline for
submission of consolidated data and report from the DILG Regional Offices is set on October
18,2019.
Related thereto, please be informed that for LGUs that are still in the process of
reorganization, incumbent PLEB members are advised to participate in the accomplishment
of the said Monitoring Tool.
‘The baseline year for the accomplishment of the tool will be from CY 2017 to 2019.
For the questions which did not indicate the baseline year, CY 2018 to current year shall be
the basis for providing the answers.
In consideration of consolidating all the data and submitting progress reports to the
regional offices, the Regional Focal Persons and PMT Heads/PLEB Project Assistants are
advised to set an earlier deadline for the Provincial Focal Persons to send their Consolidated
Data from the Monitoring Tool.All accomplished Monitoring Tool and other attachments such as: Executive
Orders/Ordinance/Resolutions relating to the PLEB Creation and Composition; and photo
documentations (scanned copies) by the LGU shall be submitted to the Regional Offices
through Provincial Offices.
The Regional Office shall upload all consolidated sheets and monitoring tools and
other necessary documents/attachments through this link: ttos://tinyurl.com/PLEBM-
£2019. All the required documents shall be labeled accordingly (Ex. Provincial: Isabela
Consolidation Sheet, Ramon PLEB Creation Executive Order; Regional: Region II
Consolidation Sheet, Region II Progress Report).
All PLEB Project Assistants /PMT Heads shall prepare an initial presentation from the
gathered and accomplished provincial consolidation sheets, regional consolidation sheets
and progress reports for the upcoming activity entitled: Presentation of Assessment
Results on PLEB Creation and Functionality to be held on October 17, 2019.

a Manual Recording and Computerized /Digital. The overall total should be the total of

using a logbook solely dedicated to complaints filed for the manual recording. The

Item No. 3: Information Contained at PLEB Logbook. Tally in accordance with the
number of PLEBs complying with the following required information in filing a
complaint/case:
a. _ Fullname, address and contact information of the complainant;
b, Full name, rank and station or assignment of the respondents; and
c. Anarration of the material facts (the place, date and time of commission of
the offense).

Item No. 4: Logbook is kept and properly maintained in a steel or any secured
cabinet. Tally in accordance with the number of PLEBs complying with the
requirement stated. The overall total should be the total of number of questionnaire
gathered or PLEBs/LGUs who complied with the Monitoring Tool.
Item No. 5: For Computerized /Digital. Tally in accordance with the number of PLEBS
complying with the following:
a. Maintains soft copy of encoded complaints/cases; and
b. Hasa database and/or software solely dedicated to PLEB related-complaints
or cases.The overall total should be the total of number of questionnaire gathered or
PLEBs/LGUs who complied with the Monitoring Tool.
Item No. 6: Issuance of Police Clear. Tally in accordance with the number of PLEBs

a, Issues Police Clearance to PNP Officials; and
