I.

Requirements on Nullity of Marriage:

 Petitioner should submit to the Court within ten (10) days from receipt
hereof a Sworn Certification (with house location sketch) issued by the
Barangay Chairman where he is residing for at least six (6) months prior to
the date of filing of the petition.

 Furnish the Office of the Solicitor General and Office of the City Prosecutor
copies of the Petition and its Annexes and submit to the Court Proof of
Compliance thereto within the reglementary period of five (5) days.

 Copy of the Sworn Certification (with house location sketch) issued


by the Barangay Chairman.

 Copies of Registry Receipt as Proof of Compliance that the copies of


the Petition and its Annexes were duly furnished to the Office of the
Solicitor General, Office of the City Prosecutor, and to the Office of the City
Civil Registrar.

II. Jurisdictional Requirements as a proof of his residency:

 Sworn Certification of Residency (with house location sketch) issued by


the Barangay;

 Sworn Statement of the undersigned counsel that he has verified


petitioner’s residency and that petitioner has been residing thereat at least
six (6) months prior to the filing of the petition.

 Government issued ID’s bearing the photograph and address of the


petitioner and issued at least six (6) months prior to the filing of the petition.

III. Additional Requirements for nullity of marriage:

 NSO/PSA Certificate of Marriage


 If applicable: “Fruit of marriage(Child)”: Copy of Certificate of Live Birth

 If applicable: Copy of Clinical Psychological Report
