Week 1(July 8-12)

I arrived at th City hall at approximately 7:30 am. It was my first day of training and theres not much to
do .I greeted cordially all the employees and they also gladly replied.Nothing is much to be done but I
tried to make a medical assistance but I failed because I'm too nervous thats why I've instead decided to
assist clients outside the office . It was a tiring but satisfying day and I'm looking forward tommorow
positively.

The next day I've arrived at 7:45 am I greeted the CSWD Acting Head Officer Mrs. Marlyn Cumba. I
assisted clients and helped them complete their requirements that day is uneventful I repeatedly
assisting clients.

The next day I finally tried to do medical assistance and suceeded but I've forgotten to get the clients
community tax certificate after I've already given her a claim stub.

The events in the next day are just the same filing requirements for medical assistance repeatedly.

Week 2 (July 15-19)

During that day, we have a task, we needed to file the medical assistance by month in consecutive order.
It was a lot and confusing because when you are not careful you can mix up things when logging it in the
logbook. You can put some files in the month of june but it is already july thats why we have a lot of
rewriting to do.

That days we just kept on assisting clients medical assistance, senior citizens , counseling and a lot more.

I also help the PWD and Senior Citizen counter on the groundfloor because they needed a little help. I
learned how to type in a typewriter and how to ID a PWD and Senior Citizen client.I also learned what
are the requirements on getting PWD and Senior Citizen ID. I also learn what are the benefits of being in
the program of PWD and Senior Citizen.

Week 3 (July 20-26)

On that day, I have a Solo parent client, I assisted her in claiming his ID.I first check her requirements
then set forth to type her ID. I have a few errors but I done it well. I ask the client to sign in the log book
and I have my first Solo parent assisted.

The next day I needed to help the PWD and SeniorCitizen office because they are short in people. There
was a lot of clients. I think we assisted 20 senior citizens needing an ID and 5 PWDs
Week 4 (July 29-August 02)

I assisted many clients during that day. All of them are medical assistance, some are solo parents and
some are needing social case study.

The requirements for medical assistance are Barrangay Indigency, Letter to mayor, Community Tax
certificate,2 valid ID, Medical Certificate, Prescription or laboratory Request.

Before the client can proceed, they needed to go to the mayors office to mark the letter with a received
stamp.

After that we will check their requirements by date thats when they can proceed in the office so that a
social welfare aide can file their assistance.

In case study, The requirements are just very simple, Baranggay Indigency, Medical Certifcate, Laboratory
Request, hospital bill, prescriptions, a valid ID and especially the refferal, some clients have a confusion
on what referral is. It is a small list that will be given to you when you are needing a social case study. It
was a list of requirements to avail medical assistance on the patients hospital. I have tried to make a
social case study but its a little difficult and has many formats.

Week 5 (August 5-9)

I have been in the PWD office and Senior Citizen. At first there are few clients but suddenly It became
plenty.

Many clients are asking what are the requirements so I have to explain to them what is it. The
requirements for it is Baranggay Indigency, Medical Certificate, 3 pieces of 1x1 pictures, community tax
certificate. After that we can process his or her I.D . The senior citizen office is also merge in PWDs thats
why I assisted also some Senior Citizens. The requirements for senior citizen ID ,Baranggay Indigency
,Community tax certificate, 3 1x1 pictures. The benefits you can get on being a senior citizen is you can
have a booklet that gives you discount.

The first booklet is DTI booklet for groceries and the other is for pharmacies. This discounts are only for
the clients.

They also have a booklet for movies and a bucket of Fried Chicken from jollibee for their birthdays.

Week 6 (August 12-16)

This week it has only a few working days.

I have half days because we have a thesis and I need it to revised. During that days I only repeatedly
assisted medical assistance clients. Even this week that have holidays, the clients are plenty.Some are
solo parent and some are senior citizen thats why we are very busy.
I have done a medical assistance. She is needing an assistance for the hospital bill. Her son have been
admitted in the hospital because of dengue . They are 6 in the family and her husband is only a
construction worker. Their monthly allowance from him is not enough to support her sons medical needs
thats why she went to this office.

They gave given her a Php 3,000 medical assistance to be claimed after 3 weeks.

Monday, August 19, 2019

I arrived at 9:00 pm at the city hall.I assisted some clients for medical assistance and some solo parent
after that I have made a medical assistance and send it to a social welfare aide to check it after some
renovations, I finally done it.

I went home around 5:00 pm.

Friday, August 23,2019

-I arrived at that day around 9:00 am in the morning. While I was checking a clients requirements for
Medical Assistance, I have told to go to assist the PWD and Senior ID counter because Mam Manriza and
Mam Sanimae left for a meeting of Women's Person With Disability Meeting at 1:30. We started to check
the requirements of the senior citizen clients for the processing of ID but Mam Myra Sanchez
announced that we don't have yet ID templates for making their Identification cards and there is a
shortage in Booklets.After that, I was told to go to the City Jail for delivering Mam Imelda's prepared
Lunch for his son, Allan Rey. I went back to the Office after my task is done. I went home around 4:45
pm.

August 27,2019

I arrived at 8:00 am and immediately assisted some medical assistance clients.After that I assisted some
solo parent in making their ID and I got confused in the ID numbering system that's why mam Lisa
revised it.

So far that events at that day are just redundant and I went home at 5:00 pm.

August 28, 2019

I have arrived at the city hall at 8 :00 am in the morning, Many clients are there already some are case
study and some are medical assiatance.

Around 1:00 oclock , Mam Imelda asked me if I can bring lunch again to her son in the city jail.
I came back around 4:00 pm after that theres not much to do. We went home at 5:00 pm.

September 02,2019

I arrived at the city hall around 8:00am in the morning. We assisted some clients medical and social case
study. We filed some medical assistance.After that, I went to PWD and Senior Citizen counter in the
ground floor because Mam Loraine was the only personal there so I helped her before Mam Sanimae
arrived.I came back and assisted some medical assistance clients.

I went home around 5:00 pm.

September 03,2019

That day I filed some AICS and assisted some. After that I assisted some solo parent ID, and filed them
neatly in the appropriate folders.

I was asked to give assistance to the elderly individuals who was asking for medical assistance.

That day, I went home around 5:30 pm.

Wednesday,September 4 2019

I arrived at 8: 15 am in the City hall. There are few clients and I just assists some solo parent clients and
medical assistance.My past client came back to proceed her medical assistance and I processed her
paper because She have already have a medical assistance form.

During lunch, There was a competition for Baranggay Best Parade I watched some Baranggay floats and
street dancers perform.

I just sorted forms of medical assistance before going home.

September 5,2019

I arrived at the city hall around 8:02. I continue to file the medical assistance forms by date and sort it
out by date.That day, there a few personnel in the office because there was an Alay lakad around 7:30
am. There are several social case study and I was happy to make one but I was asked to go to the PWD
and Senior Citizen counter because Mam Loraine is going to get a medical certificate in the clinic near
their Baranggay for her son. After a while they came back and I went to CSWD office. Mam Cora went
out and asked me if I can accompany her in PNP building nearby I gladly accepted. I assisted some
medical assistance clients after I came back.

I went home around 5:00 pm in the afternoon.

September 06,2019

I arrived at the office around 8:00 am in the morning. I continue to do the filing of medical/burial
assistance forms.After that I assist some medical assistance clients. In that day,We are celebrating the
birthday of Vice Mayor Efren Bartolome.We received 2 trays of lasagna from him

I needed to go the school around 12:30 because we have a class in Research but there are many clients
thats why I arrived around 12:40.

September 11, 2019

I arrived at the city hall around 8:06 am.I was asked if I can assist Mam Loraine in the PWD and Senior
citizen counter before going to the office. After that, I assisted medical assistance clients. After a while I
was asked to go the administration office for some receiving task.

I went back to assist some more clients.

We went home around 5:30 pm.

September 13, 2019

I arrived at the city hall around 8:00 am in the morning. I assisted some client in medical assistance.We
have a class around 12:30 thats why my time is limited. After I assisted some clients , I have a task to file
an agency profile about CSWDO I went to the office of FSCAP to request some Profile of their agency.

Before I went to the city hall, I went to the CSWDO Satellite Office in Baranggay San Martin II

I requested some copy of PWD's agency profile. I managed to get an The organizations profile and
programs and services.

After that ,I proceeded to the City hall and I went home around 5:00 pm.

September 16,2019

I went to the office around 12:00 in the morning because I went to the CSWDO Satellite Office at.
Baranggay San Martin II. My copy of The PWD's organization profile have been deleted. I tried to get a
copy again but failed because the focal person is not on duty.

I assisted some client's medical assistance. I finished some task about receiving copies in the
administration office. I have made a solo parent ID.

I went home around 5:30 pm.

September 17, 2019

I arrived around 8:30 am. Mam Loraine asked my help to assist some clients in the Senior and PWD
counter in the groundfloor. We assisted some requirements of clients requesting for ID' s and told them
that we dont have yet templates to finish their respective Senior and PWD 's IDs.
After some time ,the OSCA head arrived and I just assist them in helping clients.

I went back at the 3rd floor office and assisted some clients.

I went home at exact 5:00 pm.

September 19, 2019

I arrived at the city hall around 12:00 am.I assited some clients and made some single parent IDs.

I went to the Federation of Senior Citizens Association to follow up about their Agency profile but they
said that they dont have already the information Im seeking.

September 25,2019

I went around 10:00 am in the morning in the city hall. I assisted a couple of clients for medical
assistance.

I once assisted a single parent client for filing her requirements but it was not complete thats why we
withdraw it back to her . There wasnt an Income Tax Return and her birth certificate together with her
sons birth certificate.

I accepted her requirements but she didnt brought back her Baranggay Certification.

There was no similar events happened like that after and we went home around 5:00 pm.

September 26, 2019

I arrived at the city hall around 9:00 in the morning . We assisted some medical assistance clients .

Around 12:00 , a black out occured in the city hall.I went out to have lunch and came back immediately
to assist some more clients.

There was a client who wants to acquire a medical assistance but her medical certificate are not eligible
because of the purpose writen on it.

I assisted her to the city health office and asked if the medical certificate can be rewritten to change the
medical purpose but there was no doctor. We went to the Health center where the clients filed the
medical certificate but they also refused because the doctor that examined her son was also out in a
seminar.

I went home around 4:57 pm.

September 27, 2019

I arrived at the city hall around 8:30 am in the morning. I assisted some clients and after that I headed to
the PWD iding counter to assist some clients and help mam loraine in some senior citizen clients. I was
asked to go to the 3rd floor to do some errand for PWD counter.

Around 12:00 pm in the afternoon, I have a class in Research thats why I needed to go to school bu while
im already in the jeep thats when I have known that there was no classes becuse our professor are in
outreach.

September 30, 2019

I arrived at the city hall around 8:15 am. I assisted Mam loraine Manriza at the PWD office . We assisted
some senior citizens requirements for issuing of IDs.

After a while, Mam Zeny arrived and Mam Sanimae. Mam Zeny asked if I can get the certifications for
substitute for IDs.

I assisted a medical assistance about animal dog bite. We went to the city health office near KISAP
building to get a medical certificate and went back to the mayors office to received the letter.

October 02, 2019

I arrived at the city hall around 9:00 am. I assisted medical assistance clients and solo parents clients.

I have made a solo parent ID of a relative of some HR staff.

Mam Imelda and Mam Cora went to grace ville to rescue a child while I am in the office to assist some
medical assistance clients.

They came to office an hour later.

We do all medical assistance clients that day.

October 7, 2019

I assisted some medical assistance clients during that day. There was a parent asking for a medical
assistance but her requirements are not enough. Thats why she needed to came back.

I have made a solo parent I.D for a client.

I assisted some senior citizen looking for the special desk of PDAO/PWD and Senior Citizen in the ground
floor.

I went home around 5:00 pm.

October 8, 2019
I arrived at the city hall around 8:00 am in the morning.

I assisted some clients who is needing medical assistance. After that, I assisted some client who is
needing social case study.

After a while, I came to the special desk to assist some clients because Mam Sani mae not yet arrived.

I went home around 5:00 pm in the afternoon.

October 9, 2019

I arrived at the City hall around 8:00 am in the morning. I assisted some medical assistance clients and
some social case study. I went to the special desk in the ground floor to assist them in cleaning their
office I proceeded to the congresswomans office to process my educational assistance but I dont have a
2x2 licture thats why I decided to go back at the office. There was a client who was asking for temporary
shelter for the aged. I recommended her to go to Sta.Maria , Bulacan because was a good shelter there
but it was private and needing a monthly deposit for grandmother and grand father that was admitted
there.

October 14, 2019

I arrived around 8:00 am in the City hall. I assist medical assistance clients and some social case studies.

I escort a client to the city health center to acquire citicoline, a maintenance pills for high blood pressure
but they announced that they don't have it. The client needed it for her husband who is an alcoholic.

I assisted Mam Loraine in the PWD office.

I went home around 5:00 pm in the afternoon.

October 17 , 2019

I arrived at the city hall 8:00 am in the morning. I assisted several medical assistance clients. There is a
lot of clients on this day. There are about 20 plus clients who is needing social case study and a lot of
medical assistance. Mam Loraine askec me If I cant get a locator for her and Mam Sani mae.

It was already 3:00 am and there was still a lot of Social Case Study and Medical Assistance.

I went home around 5:00 am.
