Step 1:

Fill out the application form

Step 2:

Pay the necessary fees. As of now, the fees for

PSA birth certificate request through SM
Business Center is P155 per copy plus P25
service fee each request.

Step 3:

Claim your document at the releasing section of

SM Business Services. Don’t forget to bring the
receipt, your valid ID and authorization letter
if you have a representative.
