The Schools Division seeks to establish a systematic and continuous gathering, processing, interpreting, analyzing and storing data for management decision. It aims to continuously improve the delivery of education services to achieve the desired outputs and sustainability of education outcomes and organizational performance for efficient and effective fulfilment of the DepEd’s mandate. 2. In this lieu, this Office issues the SMEA forms to be accomplished quarterly by the School Quality Management Team (SQMT) headed by the school head. Quarterly report of each school shall be submitted to the Public Schools District Supervisor/Coordinating Principal for consolidation who will then submit the consolidated report to the SDO.
