I, ROGELIO LIM LORENZO, of legal age, Filipino citizen, married and with residence at
No. 43 Catalina Subdivision, Barangay Rosario, Pasig City, after having been sworn in
accordance with law, hereby depose and say:

That I am the surviving spouse of the deceased IRENE CAINA LORENZO as evidenced by
Marriage Contract No. 1776 (f-83) issued by the Civil City Registry of City of Manila , copy of
which is hereto attached and marked as ANNEX “A”.

That I am the sole and only surviving legal heir of the late IRENE CAINA LORENZO, since
she did not bear any child during our cohabitation, who died on February 4, 2005 at the Rizal
Medical Center of breast cancer, leaving without any will or debt that has not been settled up
to the present. Copy of Certificate of Death No. 2005-500 issued City Civil Registry of Pasig City
is hereto attached and marked as ANNEX “B”.

That the said deceased at the time of her death left a conjugal parcel of residential land
situated at Barangay Rosario, Pasig City which is more particularly described to wit;

“A parcel of land (Lot 3-B-1-B-2 of the subd. plan Psd-13-011692, being a

portion of Lot 3-B-1-B, (LRC) Psd-288168, L.R.C. Rec. No. N-4961), situated in the
Brgy. FRosario, Mun. fo Pasig, Metro Manila, Is. of Luzon. Bounded on the NE,
along line 1-2 by J. M. Tuazon; along liner 2-3 by Ambrosio Santisteban; on the
SE., along line 3-4 by Lot 3-B-1-B-3, of the subd. plan; on the SW., along line 4-5
by Lot 3-B-2-B, (LRC)Psd-288169 (Existing Road); and on the NW., along line 5-1
by Lot 3-B-1-B-1, of the subs. Beginning a a point marked “1`” on plan, being N,
65 deg. 02’ E., 610.91 m. from BLBM 31, Bo. Rosario, Pasig, Rizal, thence S. 84
deg. 29’ E., 7.25 m. to pt. 2; S. 56 deg. 36’ E., 5.60 m. to pt. 3; s. 17 DEG. 43’ w.,
16.73 m. to pt.. 4; N. 86 deg. 18’ W., 11.75 m. to pt. 5; N. 14 deg. 33’ E., 19.59 m.
to pt. of beginning, containing an area of TWO HUNDRED TWENTY SEVEN (227)
SQ. METERS. All pts. referred to are indicated on the plan and are markied on
the ground by PS cyl. conc. mons. 15 x 60 cm.; except point 2 & 3 by Old PS cyl.
conc. mons. 15 x 60 cm., bearings true; date of original survey, July 20, 1011, that
of the subs. survey, on Sept. 2, 1989. Approved on May 02, 1991.” Copy of which
is hereto attached and marked as ANNEX “C”.

That pursuant to Sec. 1 Rule 74 of the Revised Rules of Court of the Philippines, I hereby
Adjudicate unto myself the above described parcel of land without judicial proceedings as
prescribed by the aforementioned Rules of Court.
