BACKGROUND

Trauma is still a leading cause of death and has enormous impact on patient’s life and health systems.
Motorcycle accidents are the most frequent cause of multiple trauma. According to “WHO Global
Status Report on Road Safety” more than 1.2 million people die on the world’s roads every year and as
many as 50 million others are injured (WHO). In Indonesia, the prevalence of traffic accidents in 2018
was 28,262 with a mortality rate of 5,914. During 2019 until June, there have been 4,180 traffic
accidents with a mortality rate of 1,044 with a single traffic accident of 172 cases. (Pfeifer, et al, 2016).
The hospital course of patients with multiple traumatic injuries is frequently accompanied by severe
complication. Multiple trauma usually present challenging clinical scenarios with musculosceletal
injuries that are life threatetning so this case needs immediate management. Hemorrhagic shock is one
of the central problems in patients with multiple trauma and a common cause of death. (Frink, et al,
2017). In this case, the patient suffered multiple trauma due to traffic accidents, at first glance the
patient was seen only exposed to open fractures and close fractures on the right foot, but apparently the
patient suffered some other trauma such as subdural hygroma and mandibular fractures which makes
patients need special medical handlers immediately. Based on the rare occurrence of this incident, the
varied needs of treatment, and the need for close supervision of the patient in order to maintain and
improve the patient's condition wherever possible, we adopt this case to be discussed on this clinical
case presentation.
