Dear Food and Drug Administration,

I, Renia A. Lumanlan, a Registered Pharmacist and owner, would like to voluntarily

surrender the certificate of License to Operate (LTO) of ERLREN’s Drugstore and
General Merchandise located at Block 14, Lot 4, Gardenia Valley, Brgy. Molino 3, City
of Bacoor, Cavite. The reason for surrendering is that it is unprofitable , therefore, I
have decided to forgo and cease my drugstore to avoid any further loses. Thus, this
will be effective this August 2019, wherein, it will stop operating as a drugstore.
