Joint Affidavit of 2 disinterested persons attesting to the fact that the applicant and student with incorrect names/entries refer to on and the same person. Affidavit of the student, if legal age, or that of parents/guardian, if minor, explaining the circumstances that led to the use of the assumed/erroneous name or incorrect entries, and attesting to the same fact as indicated in Number 2. Letter Request for correction of name or school entries, addressed to the Regional Director, specifying the Schools attended/graduated from Elementary to Secondary, and the names/entries used. School records bearing the erroneous name/entry. Certification from the School Head justifying the incorrect name/entries. Certification of the Attorney III, or Administrative Officer, or the Supervisor-in-charge of Private Schools noted by the Schools Division Superintendent, attesting to the fact that they have validated all the documents submitted by the applicant.
