Dear Mam,

I am writing your office to inform you about the error in my teaching load this semester, SY.
2019-2020. I was asked to sign by Ms. Crishna last June, 2019 immediately for my employment
contract in your institution stating that I have 5 units load for this semester, but I was able to
double check the subjects that was loaded to me and its corresponding units and it appears that
the total units should be 7 units.

The following are the subjects loaded to me this semester:

HOSPITAL PHARMACY LECTURE 3


HOSPITAL PHARMACY LABORATORY 1
PHARMACEUTICAL BOTANY WITH TAXONOMY LECTURE 3

Also Hospital Internship equivalent to 4 units was added to me by Dean Quiogue last July. A
total of 11 units are loaded to me this semester.

I am asking for the correction of my contract and a retroactive pay for the unpaid units. Thank
you for accommodating my request.
