I, DR SISIR KUMAR MANDAL S/O SH. AHINDRA NATH MANDAL

Permanent resident of VILL+POST-TRIMOHINI, DISTT-DAKSHIN
DINAJPUR, WEST BENGAL-733155, and I am presently residing at
C-129, GOLE MARKET, JAWAHAR NAGAR, JAIPUR-733155,

1. I have been selected for the post of ASSOCIATE PROFESSOR

DEPARTMENT OF ROG NIDANA in All India Institute of
Ayurveda, New Delhi vide Letter No. F. NO.A17001/1/1/2012-
AIIA/VOL.X/2C dated 8th march 2019.

2. I certify that, the information given by me in the Attestation form,

identity Certificate and Home town declaration form, submitted to
All India Institute of Ayurveda, New Delhi is correct and complete
to the best of my knowledge and belief.

3. I undertake that if anything adverse comes out in the character

and antecedent verification or in the medical examination, that
would make me ineligible for appointed in the first instance and I
may be liable to be discharged from Government service/Institute
forthwith without any further notice
