AFFIDAVIT OF BANKRUPTCY

The undersigned, REGALADO BALUYOT y HERRERA, 75

years old, single, Filipino, and presently residing at No. 11
Aberdeen Street, Bahay Toro, Project 8, Quezon City , after
having been sworn to on oath in accordance with the law do hereby
depose and say:

1. That Affiant’s canteen business at BMEG Feeds Canteen at A.

Bonifacio, Balintawak, Quezon City ceased to operation
during the last quarter of 1996 due to business losses;

2. That affiant’s canteen at UPPC Canteen at Calumpit, Bulacan

ceased operation during the third quarter of 2007 likewise
due to business losses;

3. That in addition, affiant’s catering service ceased operation on

January 2012 when his trusted assistant Larry Hao abscond
with his hard earned money in the amount of Four Hundred
Thousand Pesos (Php 400,000.00);

4. That affiant had previously invested all his savings with these
previous businesses. At present, he does not have any real or
personal property in his name nor any savings at the bank;

5. That I execute this affidavit to attest to the truth and veracity

of the above foregoing and for all other legal purposes it may
serve.
