Endorsement to VPAA by the Dean with ff. attachments (1 set):

1. Letter Request for Curriculum Revision (Address to VPAA)


2. Endorsement by the College Curriculum Review Committee (CCRC)
with findings and recommendations
3. Minutes of Local Academic Council (LAC) meeting
4. Revised Prospectus
5. Old/Previous Prospectus with CHED Notation
6. Comparative Matrix of Curriculum Components
7. Program Structure
8. Form 2-Revision of Curriculum (Please revised the “Justification” to indicate the changes
of the program)

*If it is approved by the VPAA, you can produce 7 sets for evaluation to the UCRC.

For CHED Notation (3 Sets):

1. Revised Prospectus
2. Old/Previous Prospectus with CHED Notation
3. Comparative Matrix of Curriculum Components
4. Course Titles and Description (All Courses)

For BOR Approval:

Hard Copy (5 sets)


1. Revised Prospectus
2. Old/Previous Prospectus with CHED Notation
3. Revision Brief (Per Program)

Scanned ONLY the following documents with complete signature and send to CIO:
