Please read this service contract fully before

signing.
The undersigned agrees to hire the services of Carl
Schmid under the following conditions:
Carl Schmid agrees to perform at: Greczek/Mcrae Rehearsal Dinner

Address: Hillsboro Club, 901 Hillsboro Mile, Hillsboro Beach, FL, 33062

To provide music services described as: Violin, Guitar, Vocals on Friday,

October 15, 2010 from the hours of 6:00 pm - 9:00 pm. The undersigned will
allow one and a half hours for set-up of equipment prior to engagement.

Rate agreed upon for professional music services is $350. The undersigned
must pay a deposit of $150. The deposit reserves your date and time
specified above. Contract shall not be effective until the deposit is received.
The remaining balance of $200 must be paid in full prior to the start of the
event. If the remaining balance is not paid prior to the previously mentioned
time, Carl Schmid reserves the right to not fulfill the remainder of the
contract.

There will be a $50.00 charge on all returned checks. In the event of a

returned check, cash, money order or cashier’s check shall be the only forms
of payment acceptable thereafter.

All cancellations must be made at least 10 days prior to the agreed upon
date and time above. Otherwise, the remaining balance for contracted
services are still due and must be paid in full prior to the above time and
date of engagement. All deposits reserved for engagement are non-
refundable.
The undersigned is financially responsible for and damage(s) as a result of
any person(s) or foreign objects coming in contact with any equipment
belonging to Carl Schmid.

If the engagement is delayed or ended before the above specified time

through no fault of Carl Schmid, the undersigned shall be liable for full
payment of services rendered.

Reasonable power for equipment shall be provided and is the undersigned’s

responsibility.

Parking fees for five vehicles shall be paid upon receipt given to the
undersigned if REASONABLE parking is not available during the time of the
event.

The undersigned has read all of the above requirements

and by signing this contract is bound to these conditions.
