Guide:

1. TIN: This should match with Taxpayer Identification Number (TIN) in the sworn declaration.

2. Name: This should match with Name in the sworn declaration.

3. Registered Address: This should match Business Address in the sworn declaration. If business
address is same as permanent address, this should also match with Address.

4. Registration Date, Tax Type, Trade Name, Line of Business: Details in this part varies in each
Certificate of Registration.

5. Documentary Stamp: On the lower left part of the Certificate of Registration, there must be
documentary stamp/s attached.

Note: Some Certificate of Registration has an expiration date. Please check if the COR is not yet
expired.
