Dear SIR:

This is to acknowledge receipt of the documents submitted by your Municipal Social

Welfare and Development Officer, Ms. Remedious C. Pascual, under the partially
damage category of the Emergency Shelter Assistance (ESA) and Cash for Work Re:
Typhoon Ompong.

Upon review of the documents, we have observed some discrepancies such as, names
are included in the masterlist but no supporting documents while others have supporting
documents but not included in the masterlist provided. (Please see attached list.)

In view hereof, may we refer to you thru the MSWDO for her further review and
assessment.
