Rationale
 Principle of mutual sharing and helping among its members
 Three instances when a member would seek most the help of fellow members:
 Sickness
 Death
 Fortuitous events

Three Categories of MAB


 Medical Assistance
(Hospitalization / Home Confinement)

 Confinement of at least 3 days


 Prolonged stay at home
 Death
 Assistance to fortuitous events
 Fire, earthquake, robbery, flood...

Availment

 If allotted MAB budget for a month period is less than the total amount of MAB
applicants, WELFCO will consider the instance as multiple claims and will decide on fair
distribution with reference to hospital bill after Philhealth and depth of need of the
member.
 Membership status will not be a criterion in the availment

Limits of the Benefit


 A member could avail of the Medical Assistance twice/SY once for himself/herself and
once for his/her dependents.
 Maternity/paternity claims are not covered.
 Death aid and assistance in case of fortuitous events shall not have any limits as to
frequency of availment.

Membership and Contribution


 A member shall contribute Php100.00 per month that will be deducted from the his/her
salary 4x per school year (Php250.00 every 25th of June, September, December and
March)
 An aspiring member should submit an application on or before June 10 for the 1st
semester and December 10 for the second semester.
 Deadline for the renewal of membership shall be June 15, 2015.
*Documents such as f birth certificate (self, children, siblings), marriage contract, adoption papers and other
pertinent documents maybe requested when need arises
