Leave Application Guidelines

Introduction

LEAVES (Collabera Orientation Deck)


• Employees are entitled to have LEAVE CREDITS per year. (You may refer to the signed Contract).
• Leave credits/balance must be EARNED MONTHLY.
• VL can be continuously accumulated whereas, SL will be reset on the next calendar year.
• Unearned and Overused leaves will be considered LEAVE without pay and might be grounds for
corrective action.
• Reach out to the designated Project Lead/ Employee Relations Executive about the leave balance.

Proper filling of Leave Application Form:

1. Reach out to the designated Project Lead/ Employee Relations for inquiries about the process of
application.
2. Print out the form and fill out accordingly.
3. Seek for the designated Client’s acknowledgement, then submit to the assigned Project Lead/
Employee Relations Executive. (Fully accomplished hard copy of the application form is a MUST)
4. Leave Application can be pre-approved through Email but accomplished hard copy of the application
form should be submitted before or on the exact timeline set for the submission of Collabera
Timesheets.

Vacation Leave
• File the Leave Application Form Two (2) weeks prior to the target VL date.

Sick Leave/ Emergency Leave


• Employees should inform his/her Client Contact Person and Collabera POC two (2) hours before
their reporting time.
• It is the employee’s responsibility to notify both ends. Acquired leaves without giving any advice to
Collabera Project Lead or Client POC will mean Leave without Pay (LWOP) and might be grounds of
violation.
• For two consecutive days of Sick Leave, a valid medical certificate should be submitted no later
than twenty-four (24) hours upon returning to work. Fit to work certificate must also be presented
upon request or if necessary.

Half day/ Early out


• Employees should seek acknowledgement from the Client Contact Person and Collabera POC two
(2) hours before their reporting time or before their target time of early out.
• If a Client initiated early out advice has been announced, Collabera employees still need to seek
acknowledgement from both ends and should inform Collabera POC as well.
• Accomplished Leave Application Form must be submitted before or on the exact timeline set for the
submission of Collabera Timesheets.

Note: Compliance is a Must. Failure to comply is considered as violation of the Collabera Attendance Policy
and Code of Conduct. Thus, subject to appropriate disciplinary action.
