I. INTRODUCTION

The meeting started at exactly 9:00 AM and formally opened with a prayer. Then the
adviser greeted the attendees and introduce himselg. Based on the attendance sheet,
there were ___ out of 33 parents who joined the event.

3. Class Schedule

The adviser presented the Class Schedule and the Teacher’s Schedule of
Grade 12- TVL –Humility and Grade 12 GAS – Aristotle. This is to inform the
parents with the subject, time and the subject teachers for the S.Y. 2019-2020.

4. School and Classroom Policies:


a. Attendance and absences

The adviser noted that the parents are obliged to send an excuse letter
every time their student/s will be absent from the class. It was also added that
the students must get an admission slip from the School’s Prefect before they
will join the class the following day.
The adviser also stressed out especially to all the member of the 4P’s or
Pantawid Pamilyang Pilipino Program to encourage their children to come to
school that is because this may affect the amount the cash they receive coming
from the government.

b. Tardiness
The adviser informed the parents that the students must get admission
slip from the school’s prefect before they will join the class.
c. Incomplete Uniform

The adviser infromed the parents that students are encourage to wear
uniforms.
5. Grading System fot Senior High School Students

The parents were informed on the grading system wherein each subjects consists
of the following: Writeen Work, Performance Tasck and Quarterly Assessment. He
further emphasized that different subject areas have varied proportion of percentage
in the said components.

6. Legal Collections and Membership Fee/ Parents Donation were also presented

The above donations will be collected starting July. It was emphasized that the
collection will be facilitated by the class treasurer and immediately forward to Maam
Sheryl A. Isada.

They were informed also that each students needs to pay 50.00 pesos for the
whole year to fund the watchman in DKDIS Campus. Additional 20.00 pesos for the
band as agreed during the GPTA Meeting S.Y. 2018-2019. And lastly the athletics fee
of 25.00 for the School players.

C. HRPTA Project

The class adviser announced that supposedly the proposed project was students
working table since the senior high schools students are writing only on their lap due to
the unavailability of the tables. But due to the early mornig special announcement of the
school head, no less that Dr. Esther A. Bajan that the working tables are shouldered by
the school through the MOOE. He happily informed the parents that there is no HRPTA
project for this school year.

D. TESDA ASSESSMENT

Together with the help of Ms. Letty O. Jamito, the DKDIS Senior High School
Coordinator, the parents were informed about the necessary things about TESDA
Assesment. She stressed out the assessment fees be collected:

Assessment fee for Housekeeping - P 650.00


Assessment fee for FBS - P 650.00
Assessment fee for Cookery - P 650.00
Pre-assessment Fee for Cookery - P 500.00
Total 2,400.00

Miss Jamito also initially informed the parents about the Joint Delivery Voucher
Program or JDVP in which she told them that it is a training for the students prior to their
TESDA assessment with the help of other private school as a partner. She also informed
that the JDVP would shoulder one (1) payment for TESDA Assessment that may reduce
their obligation but for the main time she told the parents that the school is still waiting
for the progress of the said program.

The parents agreed also that they are going to pay in a staggered payment of at
least Php. 410.00 for six (6) months starting from the month of July, 2019 until December
2019.

E. SHS EXPO
The adviser together with the SHS Coordinator informed the parents
about the annual activity for the Senior High which is the SHS Expo. They discuss
to the parents that the said EXPO is considered to be an another assessment for
the students as they are going to showcase their talents and skills to be assessed
by the different stakeholders, Division Personnel and as well as the parents. To
make this event possible. They come up with an idea to that unanimously agreed
by the body to contribute the amount of Php. 250.00 and to solicit also from the
different stakeholders.

F. ASEAN
The adviser inform the parents that there will be Mr. & Miss ASEAN. The parents
agreed that to contribute in the amount of Php 91.00 for the quota and pot
money.

G. OTHER MATTERS

a. The adviser opened up about the suggestion to have a JS Promenade. It was


unanimously agreed to have JS Promenade since it is a once in lifetime experience
of the students. This will be made possible in February, 2020

b. One parent asked for the summary of total amount of contributions and was
given a breakdown below:
