AFFIDAVIT OF UNDERTAKING AS CUSTODIAN

I, ROSEMARIE FORTO VITERBO, Filipino, of legal age, married

and a resident of Brgy. Antagan, Tumauini, Isabela, after having
been duly sworn to in accordance with law, depose and state,
that:

1. I am a Barangay Kagawad of Barangay Antagan, Tumauini,

Isabela;
2. John Aldreich Isla is one of the residents of our barangay;
3. I was informed that Isla has applied for probation having
been convicted of the crime of Violation of Section 12,
Article II of R.A. No. 9165 by the Regional Trial Court Branch
22, Cabagan, Isabela and docketed as Criminal Case No.
Br. 22-4034;
4. I was further made to understand that Isla desires to be
released on recognizance pending the resolution of his
application for probation and needs a custodian for this
purpose;
5. I hereby agree to be the said custodian of John Aldreich Isla
and undertakes to bring him to court whenever required;
6. I am executing this affidavit to apprise all those concerned
of the foregoing facts and for whatever other legal purpose
it may serve.
