MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

The Agreement, entered into by the following:

The Urdaneta City University herein represented by the University President

Dr. Elizabeth A. Montero, a local city university with office address at San Vicente
West, Urdaneta City hereinafter referred to as the FIRST PARTY;

Local Government Unit (LGU) San Manuel,Pangasinan, hereinafter represented

by the Municipal Accountant Mrs.Carmencita I. Sabater, hereinafter referred to as
the SECOND PARTY; and

Julie Ann C. Cuison, of legal age, single, Filipino citizen and a resident of
Brgy.Cabaritan, San Manuel, Pangasinan hereinafter referred to as the THIRD
PARTY;

WITNESSETH

1. That the THIRD PARTY is a bonafide student of the FIRST PARTY taking up
Bachelor of Science in Business Administration Major in Financial
Managementand such curricular program requires 360 hours ON-THE-JOB
TRAINING (OJT);

2. That the THIRD PARTY shall undertake the aforesaid OJT program upon
approval by the FIRST PARTY with the company/corporation/enterprise of the
SECOND PARTY and the latter agrees to accept the THIRD PARTY for said
program;

3. That the THIRD PARTY shall submit to the FIRST PARTY a written consent and
waiver of his/her parent/guardian relative to said program;

4. That the THIRD PARTY shall not claim, demand, or receive any compensation or
benefit during the subsistence of the OJT program from the SECOND PARTY
neither shall an employer-employee relationship exist between the SECOND
PARTY and the THIRD PARTY; and

5. That notwithstanding the absence of employer-employee relationship, the THIRD

PARTY shall be under the supervision and control of the SECOND PARTY during
the period of the OJT and the latter shall submit to the FIRST PARTY the
necessary rating of the former at the end of the training.

IN WITNESS WHEREOF, we have hereunto set our hands this 15 th day of

February, 2019 in Urdaneta City, Pangsinan, Philippines.
