GENERAL RELEASE AND WAIVER OF CLAIM In connection with my participation in the ultimate frisbee games and related events

(the Activities) organized or sponsored by the unofficial Frisbee Team composed of my fellow SJS-LS students and faculty, whereby the undersigned will play ultimate frisbee or otherwise participate in the Activities, and in consideration of my being allowed to participate in the Activities, I hereby represent, acknowledge and agree as follows: 1. I am voluntarily participating in the Activities, an activity which I understand may require physical exertion. I verify and represent that I am physically fit and able to participate in the Activities. 2. I acknowledge and fully understand that each participant in the Activities will be engaging in activities that involve risk or serious injury, including permanent disability and death, and severe social and economic losses which may result not only from their own actions, inactions or negligence but the action, inaction or negligence of others (including, without limitation, unofficial Frisbee Team composed of my fellow SJS-LS students and faculty therules of play, or the condition of the premises or of any equipment used. I hereby acknowledge that my participation in the Activities is at my own risk and assume full responsibility for any bodily injury, death or property damage due to my negligence or otherwise while in or upon the specific or general area or location at which the Activities will occur. I knowingly and freely assume all such risk, both known and unknown, even those arising from the negligent acts or omissions of others, and assume full responsibility for my participation in the Activities. 3. I will comply with the stated and customary terms, conditions and rules for participation in the Activity. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest unofficial Frisbee Team composed of my fellow SJS-LS students and faculty representative immediately. 4. I hereby agree that this General Release and Waiver of Claim is intended to be as broad and inclusive as permitted by the laws of the Republic of the Philippines and the rules/policies of St. Joseph School La Salle, and, if any portion hereof is held invalid, the remainder shall continue in full legal force and effect.
