I’m applying for the position of Midwife/Nursing Attendant vacant, at Josefina Belmonte Duran
Provincial Hospital. I am Zyra C. Reodique presently employed as a job order since July 2016 to Present.

I have been assigned in various section in the hospital and have performed my assigned task diligently
and with exemplary performance. I have never been involved in any wrong doing during my
employment.

I am very qualified for the job because of my educational attainment and my gain experience since I am
