employment with Hotelcare. Once this is submitted, the company is under no obligation to
accept a withdrawal of this should you change your mind. If you have any doubts about your
decision you should talk to your Head housekeeper, Supervisor or Operations Manager before
completing this form.

Please complete all sections of this form correctly to avoid delays in receiving your last
payslip and P.45.

P.45 will be sent to employee’s Zimbra email account where they will have access to all
previous payslips for a period of 3 months.
