Subject: Letter of request for certification

Dear Atty. Villasis,

I am humbly writing this letter of request for certification that I am elected and assume
position as SK Kagawad in Brgy. Tamogong held last 2010 elections for 3 years from
2010 until 2013. The purpose of this request is for compliance of one of the
requirements needed to include my previous government service in my current
government service at Bureau of Jail Management and Penology in the position of Jail
Officer 1.

I am hoping for your positive response to grant my request for certification and if there’s
