The undersigned is writing you this letter requesting for a copy of my 2019
Income Tax Return.

I am a holder of the said ITR issued by your office. However, it was

inadvertently lost in my possession and despite diligent efforts to locate, the
same could no longer be found.

As such, being the sponsor for the overseas schooling of my sister Ms.
Gienen Joy E. Espilita, I am requesting for the issuance of a copy of the said
ITR on the purpose that the document shall be attached to the requirements
that will be submitted by my sister.
