AFFIDAVIT OF CONSUMED BOOKLET OF RECEIPTS

I, REV. OH JEONG HO, of legal age, married, and a resident of # 1370

Colorado St. Brookside Hills Subdivision Cainta, Rizal, after being duly
sworn in accordance with law, depose and state:

1. That I am the Board of Trustees Chairman of Asia Pacific Christian

College and Seminary Inc. with office address at #1 Sitio Bangkal,
Barangay San Isidro, Rodriguez, Rizal

2. That the said business had been duly registered with the Bureau of
Internal Revenue on June 19, 2007 and transferred from RDO 39 to
RDO 45 last October 18, 2019;

3. That subsequently, the said business have printed 10 booklets of

receipts numbered from 1501-2000;

4. That unfortunately all 10 booklet of receipts are all with entries and all
booklets had been consumed from booklet of receipt numbered 1501-
2000 ;

5. That we failed to reprint booklet of receipts in time due to the need to

transfer from RDO 39 to RDO 45 and some penalties and open cases
are needs to pay and accomplish;

6. As such, I am executing this Affidavit of Consumed Booklet of Receipts

to attest to the truth of the foregoing facts and for the reprinting of
receipts.
