3. Mention and Explaining the constraints that you were found in your daily work when you
were making nurse care plan for the patients in your hospital.
4. Each Group of students should choose one of the scenarios about Nurse Care Plan below:
 A 28 years old female is twelve weeks pregnant. She had cramping and bleeding
since this early morning. Her lips was so blue and she felt so weak. She had
temperature 38 degree Celsius, Pulse 80, Respiration 17, Blood Pressure: 90/60
mmHG
 A 18 years old male with case AIDS/HIV. He always feel tired and lately he easy
get prone to infection. T: 37 degree Celsius, P: 70, R: 16, BP: 100/80 mmHG
 A 45 years old male always feel thirsty and weak, he had dry skin and mucous
membrane, he also sudden weight loss. T: 37.8 degree Celsius, P: 75, R: 20, BP:
120/80 mmHG
 A 35 years old female said that she had a chest pain radiating to the left arm, back
and neck. She always had fatigue, weak pulse, shortness of breath and pain scale
of 9 to 10.
 A 14 years old female always feel nauseous and thirsty, she had dry lips and
chapped, poor skin. She also cannot walk properly when she got pain in her
stomach. Bp: 70/50, R: 17, P: 100, T: 39 degree Celsius.
