REGISTRATION FORM FOR RETAIL ASSOCIATE (RA) INCENTIVE PROGRAM

By registering to this Incentive Program, you are agreeing to the following terms and conditions:

1. You shall be entitled to an incentive fee of P50 per signed contract, P75 if with insurance and P100
if with life and ADLD. You will receive the incentive fee through bank deposit to your nominated
bank account (which bank account must be under your name):

 Contracts processed from current month (1st to last day of the month)-every 15th day of
the following month
2. HC Consumer Finance has the right to suspend / cancel the crediting of the entire amount of the
incentive if there are any suspicious or fraudulent circumstances related to the loan application.
3. HC reserves the right to change the terms and conditions of the incentive program or to
discontinue the same.
