with residence at Brgy. Capudlosan MacArthur, Leyte as my true and legal representative to act
for and in our name and stead and to perform the following acts:

To TRANSACT, FILE, PROCESS and REQUEST for my School Diploma and

Transcript of Records (TOR) at the ACLC-Tacloban;

To FOLLOW-UP, EXECUTE and to SIGN any documents necessary to perform for the
abovementioned purpose/s;

To CLAIM and RECEIVE my School Records; Transcript of Records (TOR) and

Diploma in my behalf;
