Based upon the verification, that no records of my house and lot at East Poblacion, Balo-i,
Lanao del Norte, I therefore request your good office to apply for re-issuance of Patent Title.

This inquiry copy furnished Regional Director of the Bureau of Lands, Region 10, Cagayan
de Oro City, as so to know my problem, I’m really very interested to free Patent my house and
lot no. TSI-V-125-BI pad-10-51006 to clear doubt once and for all. I quoted from SONA of
PDU30 encouraging warning that all land owners should have to register it to the government to
avoid confiscation.
