I am writing to formally request for an Office of the Bar Confidant


Clearance (OBC Clearance, for brevity), Certificate of Membership in
the Philippine Bar and Documentary stamp for the renewal of my
Notarial Commission.

I am Atty. Abel C. Montejo who was admitted to the Philippine Bar


on _________________ with Roll of Attorney Number _______________.
My Notarial Commission will expire this December 2019, thus
prompting me to file for its renewal.

In this regard, I am requesting for an OBC Clearance, Certificate of


Membership in the Philippine bar and Documentary stamp which are
requirements for the renewal of the aforesaid Notarial Commission.
