Dear Atty. Entila,

Good day Ma’am, I appreciate you taking your time to read my letter. I am
ATTY. FHYNN P. SAN JOSE and I recently passed the 20-2021 Bar
Examination this year. I was admitted to the bar on May 18, 2022 and was
subsequently given Roll No. 80043 as my roll number.

I am reaching out to you to humbly request two (2) copies of the following
certificates:
1. Certificate of Bar Membership;
2. Bar Ratings; and,
3. Good Standing (No Pending Case).

These documents will be needed in my application for Notarial Commission


here in Baguio City.

I paid the legal fees concerning these certificates through cash deposit with
the Land Bank of the Philippines, Baguio Branch as indicated in a scanned
copy attached to this letter and shall be sent to the following address:

In addition, I attached my Certificates from IBP National indicating my


Good Standing as a Lawyer and updated payment of Association Dues.
