Dear Honorable Consul,

I am writing this letter for the reason that my Baptismal Certificate and Form 137 are needed as a
supporting document for my application for Japan Visa. However, after through seeking the said
documents and checking from my personal files, the said documents could no longer be found due to
those documents were affected by the typhoon. Attached herewith are my Birth Certificates and other
documents needed for my application.
