A Certificate of admission to the Philippine
Bar
B Certificate of good standing issued by the
Bar Confidant
C&D Two members of the Integrated Bar of the
Philippines (IBP), Pasig City chapter
have vouched for her good moral
character as confirmed by the
certifications

[Certification of good moral character of

the petitioner by at least two (2) executive
officers of the local chapter of the
Integrated Bar of the Philippines where
he is applying for commission]

E Copy of the IBP receipt

F Copy of the PTR Receipt
G IBP Certificate of Good Member in
standing
H Filing Fee Receipt
I 3 Passport Size Color Photographs
(bearing signature at each of the bottom
parts) – not retouched as per the rules!
J Copy of the mark or impression of the
seal issued by the manufacturer from
whom she intends to purchase the seal
she will use once commissioned as notary
public
