REQUEST FORM
FOR
PERMISSION
TO STAY BEHIND OR TO DEPART EARLY
FROM A TASIS SCHOOL TRIP

I hereby give my permission for _________________________________________ to

depart from the official school trip to _______________________ prior to the end of the
trip, returning to TASIS separately from the school group. I have made alternative
arrangements and informed the Travel Coordinator in writing about the exact plans and
pick-up times for my child. Copies of transportation tickets are attached to this Request
Form.

I understand that there has to be a hand-over to a parent or responsible adult, which

needs to be arranged in advance.

I also agree that TASIS England has no supervisory responsibilities for the student as of the
time of departure from the group.
