1. PURPOSE
 To concern with HSE requirements on excavation work
 To ascertain safety for people during excavation work and people
surrounding
 To socialize and communicate excavation precautions
 To avoid material breakdowns
 To mitigate risks of excavation works

2. SCOPE
This procedure is applicable during Emily 1 Project. Contractors have to follow
mandatorily the company requirements in this document.

5. RESPONSIBILITIES
5.1 SITE MANAGER / PROJECT MANAGER
 To ensure the effectiveness of procedure
 To build management plan in accordance with the purpose of this document

5.2 PROJECT HSE MANAGER / SITE HSE COORDINATOR


 To control the effectiveness of implemented procedure
 To update necessary document related with the procedure

5.3 HSE SUPERVISOR / ENGINEER / OFFICER


 To maintain implementation of procedure in place
 To ensure technical requirement is on specification
 To update information of any discrepancies in implementation to Site HSE
Coordinator / Project HSE Manager
 To administer all related documentation in place

5.4 GEOTECHNICAL ENGINEER / FIELD ENGINEER


 To render as technical structures designer
 To advice HSE personnel on soil conditions following specific soil calculation
 To provide technical data of soil condition if needed by personnel

5.5 CONSTRUCTION SUPERINTENDENT / SUPERVISOR


 To supervise the implementation of HSE requirements for excavation work
 To implement Permit to Work System which cover excavation activities on
site
 To control communities of personnel surrounding excavated area ,ensuring
as per this safe condition
 To make sure the all personnel are working with the procedure
 To advice Contractors to mandatory safety control measure
 To state that new area of excavation in safe condition to be worked
 To state that dumping area is well compacted and equipment can operate
above designated soil

6. HSE ORGANIZATIONS
6.1 WORKING DURING NIGHT SHIFT
During working at night, precautions to be implemented are:
1. Adequate lighting shall be provided
2. Trench area / Excavated area shall be covered with proper lighting
3. Barricade areas which have high potential risk during night shift
4. Lighting of equipments (i.e: rotary light, reverse light, head light) must be
proper in operating
5. Reverse alarm shall be functioned during operational
6. Additional PPE: High visibility vest and traffic flashlight
6.2 EXCAVATION AREA
Excavation areas are applied at area which is described on below drawings

Excavation Technical Specification Requirements:

1. Excavation shall start from highest level of excavated area. Temporary road
access for excavator and dump truck shall be built with safely slope and bench.

3. Minimum slope degree shall be 15o


4. Swing rotation area shall be cleared from personnel
5. Long arm shall be used during excavation area is more than standard arm
6. Wood shall be provided in front of track to avoid collapsed in case excavate on
soft ground

6.2.1 Patrolling Management


Patrol will be implemented in order to monitor the effectiveness of excavation
working. Patrol will be implemented by daily basis and conducted by both of
HSE personnel and Construction personnel.

7. COMMUNICATIONS
In mean of communication on area / location, Radio is the mean of communication
way to be used.

8. CONTROL OF ACTIVITIES
8.1. Permit to Work
 Permit to work shall be registered a day before start work. The
requirement of excavation is refer to drawing, type of excavation,
equipment(s) to be used, and manpower involve on the specific of
excavation working.
 The excavation complimentary permit refers to Work Permit
Procedure. In case of confined space working, Confined space
permit work shall be implemented.
 Drawing shall be attached at permit to work.
 Specific Requirement of Permit to Work will follow Permit to Work
Procedure. Permit to Work refer to APAC-PRO-HSE-018 - Work
Permit System
8.2. Equipment Inspection / Certification
 All equipment shall be inspected, if needed by 3rd Party prior to
commencing of work. Equipment Certificates to be submitted to
Company prior to commencing work.
8.3. Operator’s Inspection Certification
 Operator working for excavation shall have permit from Indonesia
government and Company Inspector. Operator Certificates shall
send to Company prior to commencing work.
8.4. PPE
Mandatory PPE s shall be worn are classified as below:

8.5. Road Traffic Management & Flagmen
 During excavation work, 1(one) way traffic road access shall be
implemented.
 Flagman shall be used
 Flagmen shall be familiar and understand as well to communicate
with personnel in-charged.
 Flagmen shall familiar with the route of operation
 Traffic Sign shall be installed
8.6. Specific Precaution during / after adverse weather condition
 During bad weather (i.e: rain, storm,) activity shall be stopped due
to stability of ground in low level and high potential probability of
getting stroke.
 After adverse weather, soil condition to be checked for ground
stability by visual.
8.7. Specific Precaution when start works in a new area
 All personnel shall participate to tool box meeting
 All personnel shall aware the condition of new area. Figure of new
area shall be described by supervisor prior to commencing
excavation.
 Put permit to work in place. Permit To Work Procedure refers to
APAC-PRO-HSE-018 - Work Permit System.
8.8. Excavation Type
8.8.1. Sloping
Standard solutions apply for below 2 m depth, provided that excavation is
in non-water bearing ground. In all other situations, designed solution must
be used.
Confined space permit shall be applied for trenches which have more than
2 m depth. Trenches which have 2 m depth shall be discussed further at
JSA.

8.8.2. Wide / Large areas


When a trench becomes very wide, the struts themselves become long,
and bend due to their own weight. The bending reduces the compressive
forces than can be resisted. A competent temporary works designer must
perform a proper structural analysis.
8.9. Lake Dewatering
 Dewatering is important in areas that have high ground water tables, or
which do not have adequate drainage. Dewatering shall be done at the
beginning of and during construction when it is necessary to lower the
ground water table. Pumping need to be maintained to keep utility ditches
and cofferdams dry until all work is completed.
 Dewatering must be done so that the velocity of the discharged water doesn’t
scouring of the receiving area.
8.10. Dumping
 Toolbox Talk to all personnel shall be conducted including supervisor
 Install proper and adequate lighting for crossroad area
 Driver and Flagman shall be well trained
 Temporary road access shall be developed with enough soil strength

 Cross of Road shall be in curve formed.

 During dumping, truck shall be on compacted road on both of the
wheels.
 Barricade shall be developed to avoid equipment sink.

8.14. Surveying on Excavation Area


 Ladder shall be used during come on to lower ground which length
more than depth of excavated area.
 Warning sign shall be installed on top of ground during surveying.
In case of small excavation area, buddy system between person
underground and on ground shall be implemented as sign
elimination
 Surveyor’s electrical tool shall be water proof

9. ENVIRONMENTAL PRECAUTION
Pollution Prevention:
 Ensure all the equipments are inspected/checked and no leakage (event wet-
spot)
 Spill kits (any kind of absorbent; saw dust, rag/napkin) should be on place
 Refuel and maintenance of machinery are prohibited at the work site
 If emergency refuel is required, a mobile dip tray must be prepared

Pollution Protection:
Dewatering and discharging any water directly to environment surrounding or water
course is subject to written permit issued by the company

10. EMERGENCY SITUATION


In case of emergency, ERP and MERP will be applied.
