JOINT AFFIDAVIT OF LEGITIMATION

We, ____________________ and ___________________, both Filipinos, of legal ages,

married and presently residing at ___________________, Philippines, having been duly sworn in
accordance with law, hereby depose and state:

1. That we were legally married to each other on February 28, 2013 at a wedding ceremony
solemnized at the Office of the Municipal Mayor of _____________________. A copy of our
marriage certificate is attached hereto and made an integral part of this affidavit.

2. That prior to our marriage and outside of wedlock, two (2) children whom we named
“_____________” and “_______________”, who were conceived and born on December 19, 2009
and November 10, 2011, respectively, both at _______________;

3. That at the time of the conception of said children, we were not disqualified by any
impediment to marry each other, and, therefore, by virtue of our subsequent marriage, the said
children are now legitimated by operation of law, particularly Article 177 of the Family Code.

4. That we execute this affidavit to declare the truth of the foregoing facts.
