Greetings!

I am writing to respectfully to request for a Special Events Permit to put up an “Ukay-Ukay” along
19 Martyrs Street corner L. Barrios Street from October 16-January 31, 2019 as this is in
anticipation of the upcoming events such as Foundation Day of the Municipality of Kalibo,
Opening Salvo of the Sto. Niño Ati-Atihan Festival, and Holiday Season wherein there is great
demand for RTW (Ready-To-Wear) and other garments.

We have already secured the consent of Ms. Shirley L. Tan, owner of the lot wherein the proposed
“Ukay-Ukay” shall be put up.
