1. Visit any SRL Diagnostics Laboratory in India.

2. Use your Axis Bank credit or debit card at the front desk to avail the offer.

3. Get flat 20% discount on Routine Pathology Test and flat 10% discount on Routine Radiology
Tests.

4. Customer who shall be eligible to the Offer:

Axis Bank Debit Card Holders;

Axis Bank Credit Card Holders;

5. In order to avail the Offer, the Customer shall be required to transact through Axis Bank debit
card/credit card.

6. Participation in the Offer by the Customers is on a voluntary basis.

7. The Customers may avail the Offer during the Offer Period or until the stock lasts, whichever is
earlier.

8. The Offer is non-transferable, non-binding and non-encashable.

9..The Offer is not valid for void transactions.
