AFFIDAVIT OF UNDERTAKING

I, Cristalyn T. Rodriguez, of legal age, single/married, Filipino, a resident of Marulas,

Valenzuela City, and with business address at Unit E 3rd Floor Queen Rose Bldg. 911 Cor
Leon Guinto and San Andres St. Malate Manila, after having been subscribed to an oath,
depose and say:

1. That I am the incumbent President, of, Cosmos International Placement Agency Inc.
a domestic corporation organized under Philippine Laws, and a duly licensed
recruitment agency as per POEA License No. 237-LB-110618-RR issued by the
Philippine Overseas Employment Administration;

2. That as a licensed recruitment agency, we requested the POEA to issue in our favor
an authority to conduct Special Recruitment Activity on the corresponding dates
and venue

Venue : PESO Office, Pagadian City Hall, Pagadian City Zamboanga Del Sur.
Date : September 19-20, 2019. 8am – 5pm

3. That in my capacity as President of Cosmos International Placement Agency Inc, I

hereby DECLARE and ASSUME full responsibility in the conduct of recruitment
activities of the following representatives:

1. ANNA LEA P. CRUZ – Recruitment Staff

who has been authorized to represent our agency in the recruitment activities in the
above-mentioned place/s and date/s;

4. That I likewise DECLARE and ASSUME full and complete responsibilities for all
consequences which may arise in connection with the conduct of above
recruitment activities;

5. That I execute this Affidavit of Undertaking in compliance with the requirements

in the issuance of an authority to conduct special/provincial recruitment activity.
