Bangalore-560076 do hereby solemnly affirm and swear on oath as follows:

3. I State that my old pan card no. DWLPS7724F has been surrendered and after
the income tax verification allotted new pan card No. MJSPS4639E

4. Hence I am swearing to the content of this affidavit and to produce before the
concerned authorities for change of new pan card number.

5. That my state is true and correct.
