Booster Fundraiser Request
This form is to be used for all requests for having a fundraiser through the Athletic
Boosters account. All questions must be answered. All forms must be handed in to Carrie
O’Banion, President, or Irene Lelandais, Athletic Director. Requests must also come from
the head coach.

8. Will there be any funding needed to operate the fundraiser? If yes, to whom does it
need to be paid?
9. Have you requested a fundraiser on the ASB calendar (your fundraiser must NOT
be on the ASB calendar at DPHS)?
10. What will the funds earned from the fundraiser be used for?
11. Are you a current, paid up member of the Dos Palos Athletic Boosters? Funding
from non-members will not be considered.
