AFFIDAVIT

I, NOEMI N. APISTAR, of legal age, single, Filipino and resident of Confesor Drive, Cabatuan,
Iloilo, after having duly sworn to in accordance with law, do hereby depose and say:

That I am applying to the position of Legal Researcher at Regional Trial Court, Branch 76,
Janiuay, Iloilo;

That as part of the requirements for the application, I am required to submit a medical certificate,
which I cannot comply as of the moment;

That in lieu of the medical certificate, I am executing this affidavit to confirm that I am fit to
work subject to the submission of the said medical certificate;

That this affidavit is also executed to attest to the veracity of the foregoing statements;
